2026 Exemption Requests - BV State Squad Performance Program

This survey / form is for the purpose of submitting exemption requests by players/parents in the BV performance program.

An approved exemption allows a player to miss specified tournaments and/or training sessions. However, tournament participation and performance are part of the selection criteria. Players should be aware that not competing in tournaments—even with an approved exemption—may impact selection prospects.
